Hi,
I am interested in talking to aesthetic providers in this group. We offer derm and aesthetics and I would like to get some forms feedback for good faith exams, aesthetic history intake and aesthetic interest questionnaires. We were independent for 14 years, sold to PE and then re-opened back into private practice so I do not have many things I use to have including some of these forms. I would love to see what others have. |
